Evaluation of the clinical and histological features of paratracheal and hilar gland enlargement.

چکیده
Sixty-nine patients with a chest radiograph showing enlargement of the hilar and/or paratracheal lymph nodes were analysed. Ten of these, mainly Asian, were found to be suffering from tuberculosis. Of the remaining fifty-nine, who had sarcoidosis, 12% were tuberculin-positive. All the cases diagnosed as sarcoidosis had either a positive Kveim test and/or mediastinal lymph node histology compatible with sarcoidosis. 10% of cases with sarcoidosis showed evidence of uveitis and 10% showed bone cysts and there was also a high proportion of other extrathoracic lesions. From these studies we conclude that, whilst the currently accepted radiological staging of sarcoidosis serves as a convenient form of reference, these stages do not in any way relate to the clinical stage of disease, or to the histological appearance of affected lymph node tissue. There is a correlation between a strongly positive Kveim test of three or more granulomas and mediastinal lymph nodes showing confluent granulomata (predominantly of the epithelioid cell-type) with fibrosis. The presence of intraor peri-granulomatous fibrosis, or even gross diffuse fibrosis, does not seem in itself to be related to the nature of onset, clinical or accepted radiological staging of disease. However the mediastinal lymph nodes in patients with erythema nodosum were predominantly of giant cell-type; these patients were also Kveim-positive. Similar instances of giant cell predominance in the mediastinal lymph nodes were seen in each of the groups examined. Further studies are required to determine whether these cellular characteristics, or other histological features in the mediastinal lymph nodes, might be of prognostic significance.

Unilateral hilar or paratracheal adenopathy in sarcoidosis: a study of 38 cases.
Sarcoidosis is a disease of unknown cause that is characterized by the presence of non-caseous granuloma. The diagnosis is made whenever this histological finding occurs in the absence of other known causes. of non-caseating granulomas and whenever compatible clinical and radiographic findings are present. The usual chest radiographic findings in sarcoidosis are (1) bilateral hilar and occasion...
متن کاملThree-dimensional Volumetric Ultrasonography of Enlarged Adrenal Gland in Dog
Objective: Detection and evaluation of adrenal glands are complex problems in conventional ultrasonography in dogs. Using three-dimensional (3D) ultrasonography (US) in adrenal gland volume evaluation can add new potential for further adrenal studies. Design: Descriptive study Animals: a total of 10 mix breed dogs Procedures: The dogs underwent two dimensional (2D) and 3D US of the adrenal g...
متن کاملDifferentiating Immunoglobulin G4-Related Sclerosing Cholangitis from Hilar Cholangiocarcinoma
BACKGROUND/AIMS Few studies have differentiated immunoglobulin G (IgG) 4-related sclerosing cholangitis (IgG4-SC) from hilar cholangiocarcinoma (CC). Thus, we sought to investigate useful features for differentiating IgG4-SC from hilar CC. METHODS We retrospectively compared clinical, serological, imaging, and histological features of six patients with IgG4-SC and 42 patients with hilar CC. ...
متن کاملVolumetric evaluation of pituitary gland in dog and cat using computed tomography
The objective of this study was to evaluate the pituitary gland dimensions due to age and weight using computed tomography (CT) in dogs and cats. The CT images of pituitary gland were assessed in 11 client-owned dogs (six males and five females; age range, 1 to 9 years) and 15 client-owned cats (eight males and seven females; age range, 1 to 14 years) with no evidence of pituitary diseases. The...
متن کاملA Rare Case of Metastasis of Renal Clear Cell Carcinoma to the Thyroid Gland, Presenting as a Goiter Nodule, Three Years After Nephrectomy
Thyroid gland metastatic tumors are rare in clinical practice. Clear cell RCC is one of common metastatic tumors to thyroid. We here reported a case of incidentally found clear cell renal carcinoma metastasis to the thyroid gland 3 years after nephrectomy, in the thyroidectomy procedure performed for the patient due to the thyroid enlargement caused by multinodular goiter. A 65-year-old Iranian...
